Available below are some of the most common forms and affidavits used in connection with the deletion or addressing of exceptions in title insurance policies.   • Comprehensive Affidavit (PDF) view | download
    This affidavit is used in connection with both the "regular" and the Homeowners policies to delete certain exceptions and to permit some of the coverages under the latter policy to be given.
  • Foreign Person (Tax) Affidavit (PDF) view | download
    This form can be used to establish that tax withholding for a foreign person is unnecessary (i.e., that no seller is a foreign person or that the amount realized from the sale is less than $300,000 AND that the buyer will be using the property as a residence.
